RULES AND CLASS CONDUCT AGREEMENT

STUDENT CLASS CONDUCT:
1. To come to class prepared to fully participate.
2. To come to class on time.
3. To follow your teacher’s instructions.
4. To observe safety rules: horseplay and unsupervised activities are not allowed.
5. To show respect to self, other students and teachers. Failure to do so may result in dismissal.
6. To come to class properly dressed.
7. No jewelry is allowed. Hair should be secured back in a bun, short pony-tails, or braids.
8. Ballet slippers elastic, must be adjusted to fit, and bows tucked in. Tap shoes must be tied secure.

SCHOOL UNIFORM: SCHOOL UNIFORM IS REQUIRED FOR ALL CLASSES.
Please pre-pay order for dance attire with your teacher at the schools, or order via email DiRodin@hawaii.rr.com.
Shoes for ballet, tap and jazz can be purchased at Step-N-Up, or any retail store of your choice.

UNIFORM DESCRIPTION:
Mom & Me ages 1 to 3
For girls: School logo pink leotard and skirt, pink ballet slippers, white socks or pink tights (pink tights are required for shows).
For boys: White school T-shirt, black shorts, white socks, black ballet slippers.

Ballet -Tap ages 3 to 4, and 4 to 5:
For girls: School logo pink leotard and skirt, pink ballet slippers, white socks or pink tights (pink tights are required for shows), and black tap shoes.
For boys: White school logo T-shirt, black shorts, white socks, black ballet slippers and black tap shoes.

Ballet –Tap – Jazz ages 5 to 8:
For girls: School logo blue leotard and skirt, white ballet slippers, white socks or white tights (white tights are required for shows), and black tap shoes.
For boys: White school logo T-shirt, black shorts, white socks, black ballet slippers and black tap shoes.

Ballet Intermediate/Advanced and Lyrical: School logo black leotard and skirt, pink ballet slippers, and pink tights (Point shoes are by teachers’ approval only.)

Jazz, Precision Funk, Hip-Hop:
School logo jazz pants, school logo white T-shirt, and black jazz shoes.

Gymnastics – For girls: leotard and trunk can be purchased at the school. For boys: athletic short and school logo T-shirt.

Gymnastics Team – see Di Rodin for uniform

Cheerleading- see your coach for uniform for practice and for team uniform

Hula – see your teacher for hula practice skirt and costume.

Costume for shows – see order form and price

Dance shoes are not to be used outside the dance studio floor.
OUTSIDE SHOES ARE NOT ALLOWED INSIDE THE CLASSROOM


TUITION, ABSENCES, AND MAKE-UP RULES

9. Monthly tuition is based on 4 weeks. If a month has 5 weeks, there is no class during the last week, unless a make-up class is scheduled.
10. Tuition is due the 1ST class of the month and paid at the Hickam Community Center.
11. Student must present monthly receipt to the teacher.
12. Make-up classes are available for emergency reasons within one month of missed class.
13. Please call, and notify Di Rodin 479-3273 in case of: absence, make up class, vacation, prolonged absence, discontinue classes temporary or moving.
14. Tuition will be charged, unless notified of changes.
15. Student’s enrollment in class is kept only if tuition is paid.
16. No show or no payment for one month removes the student from the class list.
17. Monthly Tuition is non-refundable, not prorated, and credit is not given for missed classes.

CLASSES SCHEDULE POLICIES:
18. Classes are grouped according to age and skills, depending on enrollment.
19. Six students are necessary to form a class. In the event of low enrollment, we will hold a class for the first month; thereafter we may combine classes based on age or skill level.
20. If 2 or fewer students are in class, class duration will be ½ of its normal time.
21. The school offers private, small group classes and choreography. You may inquire with Di Rodin, the director of the Dance Movement Academy.
